About this role
Job Description Position Summary: This position will include client intakes and on-farm assessments for farmers, ranchers, and agriculture workers experiencing limitations due to an injury or a disability. This position will also be responsible for collecting all demographic and data collection for program evaluation, writing DVR referral assessments, AT recommendations, and making tailored client referrals.
The primary office location for this position is in the University Square building (1311 South College Avenue) in Fort Collins, Colorado on campus at Colorado State University. While this position is field based and eligible for hybrid work arrangement, the individual in this position is expected to meet weekly with their supervisor. The work schedule will vary, and frequent evening and weekend work may be necessary. Time may be flexed to maintain a 40-hour work week.

Essential Job Duties: • Manages a caseload of farmers/ranchers and/or their family members and/or individuals employed in the agriculture arena consisting of all disability areas and coordinates the utilization of multiple healthcare related or agricultural related community agencies. • Conducts participant assessments on participants involving the use of specialized knowledge and applications associated with social work, rehabilitation, psychology, or occupational therapy. Develops, in cooperation with other staff, a plan of care for each individual participating in the program. • Offers solutions to participants to mitigate or remove physical, environmental, and societal barriers that prevent participants from maintaining their independence and self-sufficiency. Provides resources for Behavioral Health when needed or requested. • Visits service agencies, attends meetings, and confers with subordinates to obtain additional information concerning community resources for persons with disabilities. • Submits Rehabilitation Technology evaluations to the Colorado Division of Vocational Rehabilitation on an as-needed basis. • Develops and delivers educational workshops on topics relevant to farmers/ranchers, their families, outside professionals, and other interested parties. • Markets the program to healthcare providers, agricultural producer organizations and individuals to provide education and increase program enrollment. • Submits monthly, quarterly and annual reports as required by Colorado State University and the National AgrAbility Project (NAP). Manages and is accountable for program data reporting including the design and coordination of data systems and procedures. Provides data outcomes for reporting program performance through dashboards, Annual Statistical Reports (ASRs), etc., executive team, board members, stakeholders, and partners.
Conditions of Employment: • Pre-employment Criminal Background Check (required for new hires), travel. • Must have a valid driver’s license or the ability to obtain a driver’s license or access to a licensed driver by the employment start date. • The successful candidate must be legally authorized to work in the United States by the proposed start date; the Office of Engagement and Extension will not sponsor a visa for this position.
Supervision: This position has no supervisory responsibility.
Minimum Qualifications: • Completed bachelor's degree in Occupational Therapy, Public Health, Education, or a closely related field. • 3+ years of professional experience relevant to the Essential Job Duties of this position.
Preferred Qualifications: • Completed master’s degree in Occupational Therapy, Public Health, Education, or another closely related field. • Demonstrated ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents based on written application materials. • Demonstrated ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community based on written application materials. • Demonstrated ability to generate appropriate response to both internal and external audiences during crisis situations based on written application materials.. • Demonstrated ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists based on written application materials. • Demonstrated ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form based on written application materials. • Knowledge of MS Word processing software; MS Excel spreadsheet software; and Contact Management systems. MS PowerPoint and presentation creation skills required. • Current CPR certification. • Course work and/or professional experience must provide a broad and significant understanding of physical and cognitive disabilities, occupational therapy interventions, assistive technologies and available resources and interventions for people living with a disability.
