Summary of Job Duties: **Employee will be located in the southern region and have a desk space at the South Arkansas Women’s Clinic in El Dorado, Arkansas**

The responsibilities of the Community Health Worker I include coordinating outreach and engagement activities, using a community-informed approach. Participates in design, implementation, and interpretation of community-level assessments (e.g. windshield survey of community assets and challenges, community asset mapping).

Presents at local agencies and community events to share information and educate individuals and communities about health and social service concerns and resources. Assists individuals in building and expanding their personal capacity to identify and manage their health conditions, obtain services as needed, identify opportunities to help others, and represent their needs through communication and advocacy. Qualifications: • High School diploma/GED plus two (2) years of general experience in community outreach, education, case management, and administration required.

• Must enroll in a certified CHW Training Program within 45 days of employment.

• Must have good organization and time management skills and have effective communicationskills.

• Arkansas Community Health Worker Association (ARCHWA) Certification is preferred.

Responsibilities: • Work one-on-one with low, moderate and high-risk, uninsured and underinsured clients, including pregnant and postpartum women at risk for hypertension in pregnancy.

• Maintain an advanced level of knowledge and assist clients in applying for Medicaid, Medicare and/or financial or medication assistance programs as appropriate.

• Connect clients to primary care/medical homes as appropriate.

• Ensure provider instructions are understood and appointments are kept.

• Educate clients on proper use of emergency room and other appropriate provider alternatives.

• Track and report patient encounters and maintain required records as directed.

• Track contact with research participants, types of education, resources, and support provided using a provided Implementation Support Tracking Tool.​

• Actively participate in available training and professional development activities, including provided training specific to perinatal health and blood pressure monitoring.

• Obtain CHW Core Competency training in the first six months of employment.

• Maintain regular communication with caseload of clients, including weekly check-ins with research participants.

• Guide clients to be active and engaged in their health and well-being, including educating research participants on the importance of home blood pressure   monitoring.

• Make follow-up visits by telephone and in-person to ensure best possible patient outcomes.

• Assist clients in building their personal capacity to manage chronic health conditions .

• Collect and distribute materials to assist the community.

• Perform other duties as assigned.
