Role Scope : The Social Work and Advocacy Supervisor is a mission-driven leader responsible for overseeing the daily operations of the Social Work team, Participant Advocates, and behavioral health therapists within the PACE Center. This role champions person-centered care by ensuring participants’ social, emotional, and advocacy needs remain at the forefront of care planning and service delivery. The supervisor provides leadership to direct reports, partners closely with interdisciplinary leaders, and ensures services align with PACE standards, regulatory requirements, and the unique goals of each participant.
Core Responsibilities & Expectations for the Role
• Lead and supervise the Social Work, Participant Advocacy, and Behavioral Health teams, ensuring high-quality, participant-centered service delivery.
• Provide coaching, guidance, and performance management to direct reports to support team effectiveness and professional growth.
• Oversee participant advocacy and the participant enrollment process, fostering trust, engagement, and a positive transition into the PACE program.
• Serve as the primary escalation point for complex participant concerns, including grievances, behavioral issues, and social service needs.
• Partner with the Interdisciplinary Team (IDT) to assess participant needs, support care planning, and drive resolution of complex social and family dynamics.
• Coordinate team staffing, scheduling, and daily work assignments to ensure consistent service delivery and operational effectiveness.
• Build and maintain relationships with community organizations, government agencies, and external resources to support participant needs.
• Promote a culture of safety, dignity, belonging, and respect for participants and team members.
• Drive continuous improvement by seeking feedback, identifying opportunities for enhancement, and supporting changes that improve participant outcomes and experience.
• Model and reinforce Habitat’s values in all interactions, decisions, and leadership practices.
• Perform in-depth reviews of participants at risk for disenrollment and coordinate with the Interdisciplinary Team (IDT) to evaluate participant needs, develop interventions, and determine the PACE program's ability to meet those needs.
• Performs other related duties as assigned.
Required Qualifications:
• Master’s degree in social work (MSW) or an equivalent combination of education, licensure, and experience in social work, behavioral health, or a related human services field. Must meet all applicable state requirements, including any required licensure or certification to practice as a Social Worker (e.g., where required by state law).
• Minimum of 2 years of experience leading, supervising, or providing oversight to social work, care management, behavioral health, or participant-facing teams.
• Demonstrated knowledge of the psychosocial, behavioral, and advocacy needs of frail older adults and individuals with complex medical and social needs.
• Working knowledge of community-based services, government programs, and external resources that support older adults and their caregivers.
• Ability to prioritize work, exercise sound judgment, and independently manage competing responsibilities within established guidelines.
• Demonstrated ability to collaborate effectively with interdisciplinary teams to support participant-centered care planning and problem resolution.
• Strong communication, coaching, and relationship-building skills with the ability to navigate sensitive participants, family, and team member situations.
• Knowledge of PACE, value-based care, integrated healthcare delivery models, or other highly regulated healthcare environments preferred.
• Current CPR/BLS certification or ability to obtain certification within 30 days of employment.
• Valid California driver’s license, reliable transportation, current auto insurance as required by state law, and ability to meet organizational driving requirements.
• Proof of medical clearance, including TB screening, and compliance with all required immunization and occupational health requirements.
