Position Summary The Clinical Nurse Coordinator serves as an experienced clinical resource within the ambulatory care coordination team, serving as a clinical liaison between physicians, patients, families, pharmacies, and external healthcare facilities to support safe, coordinated, and comprehensive patient care.

This hybrid role is responsible for managing patient communication, performing clinical triage, coordinating follow-up care, and supporting providers through timely communication of results, treatment plans, and clinical recommendations within a multidisciplinary care team.

Job Description

Primary Duties & Responsibilities:

• Manage patient communication across telephone and electronic health record systems (including MyChart), performing clinical triage, symptom assessment, scheduling and follow-up coordination, communication of results and provider instructions, and escalation of urgent concerns. • Coordinate multidisciplinary care across physicians, advanced practice providers, patients, caregivers, pharmacies, specialists, and community resources to ensure continuity of care. • Process provider-directed clinical workflows, including medication refills, laboratory and diagnostic orders, referrals, and external medical record retrieval and review. • Complete patient documentation and required forms accurately within the electronic health record (EHR) and facilitate other electronic workflows • Provide patient and family education related to treatment plans, medications, preventive care, and chronic disease management, with emphasis on complex geriatric populations. • Serve as a clinical resource and support to other nursing staff regarding triage decisions, prioritization, and care coordination processes. • Maintain compliance with HIPAA and organizational policies, participate in quality improvement initiatives, and maintain required clinical competencies and certifications. • Perform other duties as assigned.

Required Qualifications

Education: Associate Degree In Nursing Or Diploma In Registered Nursing Certifications /Professional Licenses : The list below may include all acceptable certifications, professional licenses and issuers. More than one credential, certification or professional license may be required depending on the role.

Basic Life Support - American Heart Association, Basic Life Support - American Red Cross, Registered Nurse - Illinois Department of Financial and Professional Regulation, Registered Nurse - Missouri Division of Professional Registration Work Experience: Nursing (2 Years) Skills: Not Applicable Driver's License: A driver's license is not required for this position.

More About This Job Required Qualifications: • Graduate of an accredited nursing program with two years of nursing experience. • Current RN licensure in the state of Missouri and/or Illinois, depending on location, in accordance with the Compact Act. • Basic Life Support Certification or obtains certification within one month of employment. Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements.

Salary Range $64,500.00 - $109,900.00 / Annually

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
