This position is responsible for assuring that standards of care delivery are being monitored through the Quality Assessment Performance Improvement Process (QAPI). Works in coordination with Home Health or Hospice Quality Manager to ensure comprehensive assessment accuracy, identify areas needing improvement, designing performance metrics, developing data collection tools, data display and staff education related to home health/hospice eligibility, comprehensive assessment guidance, and quality outcomes. ESSENTIAL FUNCTIONS: 1. Evaluates responses to comprehensive assessments to ensure compliance with CMS guidance. 2. Performs audits on high-risk, high-volume or problem prone areas of service. Monitors and evaluates clinical audit outcomes, identifies trends and potential improvement opportunities, recommends and conducts performance improvement activities. 3. Identifies appropriate improvement methodologies to achieve PI objectives. Develops and maintains documentation of committee meeting minutes, progress toward objectives, barriers, and results. 4. Acts as a resource to staff regarding home health/hospice eligibility, regulations, care planning and medical record documentation. 5. Communicates and collaborates with Quality Manager, Clinical Managers and VP of Home Health/Hospice to ensure the delivery of quality care. 6. Follows efficient and effective mechanisms to communicate opportunities for improvement to staff and trends to Clinical Managers and VP of Home Health/Hospice. 7. Coordinates educational offerings on topics related to documentation, resource utilization, performance improvement and associated performance metrics. 8. Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information. 9. Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. 10. Demonstrates/models the Network’s PCRAFT values during interactions with all customers. 11. Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety. 12. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices 13. Complies with Network and departmental policies regarding attendance and dress code. 14. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
OTHER FUNCTIONS: 1. Works cooperatively with Diagnosis and Admission Specialists to ensure smooth department work flow. 2. Home Health QRC collaborates with Quality Administrative Assistant to submit OASIS data to CMS in accordance with Medicare COP requirements. 3. Hospice QRC submits HOPE data to CMS in accordance with Medicare COP requirements. 4. Participates in Professional Advisory Committee and other VNA committees as assigned. 5. Participates in Network committees as assigned. 6. Other related duties as assigned ADDITIONAL REQUIREMENTS: 1. Comprehensive knowledge of professional nursing or rehabilitative theory principles. 2. Knowledge of the organization and administration of Agency programs. 3. Comprehensive knowledge of the teaching-learning process and the ability to effectively teach. 4. Ability to function as a resource and role model/mentor and ability to provide consultation. 5. Ability to accept and utilize supervision. 6. Working knowledge of computers, and ability to adapt and expand use of new programs. 7. Knowledge of State and Federal regulations as well as Accreditation Standards. 8. Knowledge of the team process and ability to facilitate personal growth and development of team members. 9. Knowledge of performance improvement principles and methodologies. 10. Ability to prioritize and problem solve. 11. Possession of a current license to practice (RN or therapist) in the Commonwealth of PA. 12. Possession of a current driver’s license. 13. Must have access to a car for performance of responsibilities. 14. Participate in annual education events or days.
EDUCATION: A bachelor’s degree in nursing or therapy from a college or university program approved by a recognized professional approving body. Home Health OASIS Certification required within 18 months.
TRAINING AND EXPERIENCE: Minimum 3 years Home Health/Hospice experience. Home Health/Hospice Quality Coordinator experience preferred. One year of experience in performance improvement preferred.
ADDITIONAL REQUIREMENT: Knowledge of Microsoft Office Products.
