Job Description Responsibilities: The Virtual Primary Care Physician (MD or DO) provides high-quality, comprehensive, relationship-based, longitudinal primary care to a panel of patients via a telehealth platform.

Jefferson’s Virtual Primary Care (VPC) practice is nationally recognized for offering best-practice primary care services, in an all-virtual environment*. The VPC practice is embedded within Jefferson Primary Care as a regional, non-profit academic health system dedicated to the needs of diverse communities, and is aligned with best-practice primary care quality and experience goals.

Physicians work within an interdisciplinary team including a dedicated practice manager, practice staff, integrated behavioral health services, population health supports, and social work.

VPC physicians, while focusing on empaneled patient care, also may assist with providing telehealth-based clinical care to patients not empaneled to them, such as transitions of care visits or supplemental access visits for practice-based primary care patients.  

*Please note that while this position is remote, there is a strong preference for candidates who reside in PA, NJ or DE.

Overview: • Primary Care for people ages 18 and older via telehealth environment • Comprehensive approach to care including preventive services, screenings, immunization recommendations; chronic disease diagnosis and management; acute and complex care needs • Whole-person approach to care, partnering with colleagues in integrated behavioral health, social services, and lifestyle medicine teams as appropriate • Collaborative approach to interprofessional team care • Performs and documents physical exam leveraging approaches appropriate to telehealth. • Orders, performs, and interprets appropriate diagnostic testing, and responds to results requiring further evaluation or investigation. • Formulates differential diagnoses by priority. • Engages/Collaborates with collaborative physicians when needed (i.e. diagnosis in question, patient not responding to treatment, etc.).;coordinates with and refers to specialists and/or a higher level of care as needed. • Complete professional billing in timely and accurate manner, in compliance with regulations.

Formulates Plan of Care • Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address preventive care, acute care, and chronic health care needs. • Utilizes evidenced-based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum. • Implements and modifies plan of care based on patient response, diagnostic testing, and specialist recommendations. • Prescribes diagnostic strategies and therapeutic interventions (as above).

Documentation • Telehealth visits are documented in EPIC system according to good clinical practice and compliance, regulatory, and billing requirements  • Provides timely documentation in Epic; meets compliance standards; and captures patient history, presenting problem, physical exam within limits of virtual care, diagnosis(es) and treatment/plan of care. • Signs/closes visit notes within 24 hours of the patient encounter.

Other Responsibilities • Reviews results of diagnostic studies in a timely manner and communicates follow-up plan to the patient • Reviews and addresses telephone and Epic portal messages (In-Basket) • Reviews specialist consults, recent inpatient summaries, and other patient information as appropriate • Cross-covers patient care needs as needed when other providers are out-of-office • Shares telephonic on-call responsibilities with practice team

Communication and Collaboration • Maintains ongoing communication and collaboration with office staff and interdisciplinary health care team. • Facilitates and communicates with patients, patient care team, and staff • Reports compliance concerns or patient safety issues/events.

Quality and Productivity • Engages with population health team to improve patient access and outcomes. • Conducts accurate clinical documentation and coding • Address care gaps, value-based care metrics, and other priorities for quality improvement as established by Jefferson Primary Care   • Meets target productivity, measured by work RVUs (relative volume units)

Professional Practice • Demonstrates Professional Practice behaviors • Engages in ongoing professional education to remain up to date in clinical knwoledge • Completes annual required health system learning activities • Maintains national board certification, state licensure and required continuing education, and membership in a professional organization as appropriate • Opportunities available to engage in research and education as per physician interest

Qualifications • Qualified applicants will hold a MD or DO degree, or equivalent • Board certified, or eligible, in Family Medicine or Internal Medicine. • Preference for at least 2 years of post-residency clinical experience • Licensure in PA and NJ, or ability to obtain by start date • Demonstrated commitment to health equity, to excellence in care including for medically and socially vulnerable populations, and to optimal patient experience • Candidates from backgrounds under-represented in medicine strongly encouraged to apply • Fluency in other languages, including Spanish, is welcome though not required.
