Job Description:

Our model is designed to help solve access, quality, and cost challenges by utilizing a blend of human encounters, interdisciplinary collaboration and state of the art technology.
The Position : • 1.0 FTE opportunity – 36 patient facing hours/week • 18 patients per day • APP supervision requirements • Equitable call rotation • EHR - Elation • Additional clinical duties/expectations/requirements
Qualifications : • MD or DO degree from fully accredited medical or osteopathic school of medicine • Successful completion of Family Medicine residency • Board Certification or Eligibility in corresponding specialty • Physician must meet employer credentialing standards
Salary Range:
