- Schedule: Part-time flexibility (pick up visits based on your availability; weekdays and/or weekends as needed) Minimum of 10 points per week.

- Compensation (Productivity Points): $58.00 / point | $75,000-$100,000/year

THE MISSION OF THIS ROLE

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

OBJECTIVES

1. Carry a high-quality caseload: Minimum of 10 productivity points/week at target visit quality.

2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

WHAT YOU'LL OWN

- Deliver skilled PTA visits in the home under the supervising PT’s plan of care.

- Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

- Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.

- Drive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving.

- Coach patients and caregivers with practical home programs that actually get done.

- Document promptly and thoroughly — defensible, efficient, and consistent.

REQUIREMENTS

- Active Michigan PTA license (in good standing)

- Reliable transportation, valid driver's license, auto insurance

- Home health experience strongly preferred

- Able to carry/transport basic therapy equipment as needed

- Competency in core home health PTA scope - Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations - Strengthening, functional mobility, and home safety assessment/modification education - Patient/caregiver coaching with practical home programs
