Location:  Based out of 330 Monroe Avenue- Covering the West team areas of Greece, Spencerport, Gates, Chili and SW city

Hours Per Week:   20 hours per week

Schedule:   8:00am-5:00pm Monday through Friday

Position Summary: Under the direction of a NYS licensed Physical Therapist, the Physical Therapy Assistant (PTA) provides and documents physician ordered occupational therapy services. The PTA provides superior customer service by modeling the Unity Brand Promise and Core Values. 

Key Responsibilities: Within standards of practice established for a PTA, completes and documents (within agency standard) related to patient care including visits notes, renewal orders and verbal orders. Includes progress or lack of goal achievement in all charting. 

• Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment. Confers with therapist directly, physician and/or interdisciplinary team members regarding patient response to therapy and any concerns.  • Provides PT gait and ambulation training. Instructs in the use of assistive devices.  • Provides instruction in safe transfer using appropriate body mechanics and equipment.  • Participates in discharge planning, provides timely and adequate patient and family teaching to facilitate discharge plan.  • Communicates regularly with Physical Therapist who oversees patient’s plan of care.  • Coordinates patient schedule with team members, readily and effectively adjusts schedule to meet changes in census, patient needs.  • Maintains professional growth through active participation in continuing education programs, departmental in-services, etc.  • Communicates pertinent information to patient and family members including goals, procedures, progress, etc. as appropriate.  • Displays the capacity for motivating others. Recognizes the effects of his/her own non-verbal behaviors upon others. Correctly interprets and responds to non-verbal behavior of others

HHCS Only: Evaluates home environment for changes needed to increase patient’s independence. Recommends equipment and assistive devices (ramps, bathroom aides, other adaptive equipment) as appropriate.

HHCS Only: Maintains required agency visit productivity.

Minimum Qualifications: Associates Degree from an accredited Physical Therapy Assistant program. 

HHCS Only: Minimum of 2 years post-graduate experience as a PTA in an acute care setting and/or an acute rehabilitation facility. 

Rehab Only: Successful completion of all fieldwork requirements and eligible to sit for PTA or OTA exam.

Reads, writes and speaks English.  Must demonstrate computer proficiency.  Excellent interpersonal and communication skills.  Required Licensure / Certification Skills: Current NYS PT Assistant licensure required.

EDUCATION: AS: Physical Therapy (Required) LICENSES / CERTIFICATIONS:  PTA - Physical Therapist Assistant certification - New York State Education Department (NYSED)New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

14607 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
