Banner Home Health is a four-star home care agency with a track record for quality and excellence and is the largest skilled home care agency in Phoenix. We provide support to our therapy teams with a robust clinical support team: a field preceptor, an IT specialist, scheduler, therapy supervisor, and nurse manager. Our agency strives for the best possible patient outcomes with support from QI specialists, documentation support specialists, telehealth resources, and more. Having a strong clinical support structure is important to supporting our field clinicians in managing their caseload appropriately while not feeling alone. As a Banner Home Health Physical Therapist, you will enjoy working in sub-assigned geographic territories with smaller travel distances than many other agencies. You will enjoy an autonomous but supportive work environment and have the option to work pay per visit or hourly. We offer mileage reimbursement and will provide you a company cell phone and a tablet for documentation in our electronic medical record system (Home Care Home Base).

SCHEDULE: Training Core: • Monday-Friday
• 8:00am-5:00pm
• First 30-60 days
After Training: Option 1: Full Time • 40- 32 Hours per week
• Monday- Friday “Work your way” 4 10hr shifts or 5 8hr shifts
• Saturday & Sunday + other days during the week “Work your way” 4 10hr shifts or 5 8hr shifts
• Commitment to working every other weekend with working weekdays on the off weeks, or working every weekend with flexibility of working one week day.
Option 2: Part Time • 20-28 Hours per week
• Monday-Friday: 5, 8, 10 hour days
• Saturday- Sunday +1 other day 5, 8, 10 hour days
Option 3: Weekend • Saturday and Sunday (possibility of other day if wanted)
• 10 Hours

POSITION SUMMARY This position is responsible for the evaluation, planning, administration and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.
CORE FUNCTIONS 1. Evaluates patient's functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals. Completes comprehensive evaluation within the established time frame set by the department. Identifies and prioritizes key limiting factors. Establishes treatment goals that are functional, measurable, patient related and reflects the key limiting factors. Collaborates with patient and family/caregiver when setting goals. Initiates discharge planning. Recommends additions to or modifications of referring orders.
2. Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs. Provides skilled treatment in a safe manner. Assesses ongoing effectiveness of treatment and modifies plan of care as indicated. Involves patient and family/caregiver in the treatment and decision making process. Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
3. Documents according to professional practice guidelines set by regulatory agencies. Documents legibly. Completes documentation within the established time frames of the department. Documents evidence based and individualized treatment techniques/plan. Documentation reflects skilled intervention and comprehensive assessment of the patient's pro
