About this role
- Role: Physical Therapist (PT)
- Schedule: Full-time Monday–Friday + occasional weekends (as needed)
- Location: Field Based
- Compensation (Productivity Points): $75.00 / point | $100,000 - $130,000 /year

The PT role supports that mission by delivering safe, reliable skilled care in the home within PT scope — with excellent communication and escalation to the clinical team so patients stay safe and on track.

- Smarter charting: Record or dictate visits with AI Scribe, start with prefilled assessments, extract medication lists from photos, and keep all visit information in one easy-to-use place.
- Scheduling that protects your time: Text patients, plan more efficient routes, and stay aligned on visit changes through one clear, coordinated schedule.
- Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.
- Complete comprehensive in-home assessments and deliver skilled PT visits.
- Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
- Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
- Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
- Manage treatment and outcomes directly, including PTA supervision where appropriate.
- Coach patients and caregivers with practical home programs that actually get done.
- Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

REQUIREMENTS
Must-haves
- Active Texas PT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver’s license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health PT scope - Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations - Strengthening, functional mobility, and home safety assessment/modification education - Patient/caregiver coaching with practical home programs
Nice-to-haves
- Prior home health experience
- Homecare Homebase (HCHB) experience (or similar home health EMR)
- Deeper clinical experience in high-signal PT populations
- Experience supervising/mentoring PTAs and collaborating on care plans
- Strong organization and time management
