Role Scope : We are seeking a Physical Therapist to join Habitat Health at our South LA PACE Center in a part-time capacity . The ideal candidate will work 16-24 hours per week . Specific schedule to be determined at offer.
The Physical Therapist provides physical therapy evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants, developing individualized treatment plans, conducting home visits as needed, and educating participants and caregivers on exercise programs and DME use. As a core member of the Interdisciplinary Team (IDT), this role contributes clinical assessments and care coordination that inform whole-person care planning, while maintaining documentation that meets all regulatory requirements. Beyond direct care, the Physical Therapist serves as a subject matter expert for physical therapy program standardization, team education, and quality audits, supports DME inventory management, and upholds safety and infection control standards — all while contributing to a center culture grounded in Habitat's values and participant belonging.
Core Responsibilities & Expectations for the Role
Participant Care
• Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong.

• Provides physical therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.
• Participates in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination.
• Assesses condition, creates a treatment plan and discusses implementation of therapy with participants and caregivers.
• Makes home visits as needed and documents activities including participant/caregiver response in participant’s chart.
• Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of DME.
• Documents therapy services provided and performs department record-keeping in keeping within documentation requirements.
Administration and Program Development
• Continue to raise the bar. Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants.
• Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants.
• Implements physical therapy program, completes evaluation and care plans timely. Oversee group maintenance exercises, including documentation by rehabilitation aides.
• Maintains the quality assurance, safety, environmental and infection control programs.
• Performs data collection, prepares reports and maintains any necessary participant statistics.
• Attends all staff meetings and patient care conferences as scheduled.
Required Qualifications:
• Bachelor’s Degree in physical therapy
• Completion of a physical therapy program accredited by the American Physical Therapy Association (APTA), the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association or other equivalent organization approved by CMS.
• Active Physical Therapist license issued by California.
• A minimum of one year experience working with the frail or elderly.

Nice to have:
• 2+ years as a practicing PT
• Doctorate of Physical Therapy (DPT)
• Bilingual: Spanish/English strongly preferred; other languages a strong plus.
• PACE experience and/ or ADCH/CBAS experience.
• A state issued driver’s license, personal transportation, and auto insurance as required by law.
