Job Description Navigation Responsibilities and Job Functions: For Patients with BI-RADS 4’s and 5’s on breast imaging: • Track the current diagnostic imaging schedules to ensure that all BI-RADS 4/5 patients are followed. • Participate in appropriate Teams chats with Imaging Techs and Radiologists regarding BI-RADS 4/5 patients.
• Accompany the radiologist (as requested) in their initial meeting with the patient, listening to recommendations made to the patient. Meet or speak with the radiologist prior to discuss the findings. • Meet with or call the patient after radiologist speaks to the patient, to provide education regarding need for breast biopsy, support, and referral for surgical appointments/radiologist biopsy.
• Make an appointment for the patient to see one of the breast specialists or with one of the radiologists for direct biopsy. Works with either the Breast Center or Radiology schedulers to facilitate/expedite an appointment as needed. • Provide teaching about the biopsy procedure and ask the patients about blood thinning medications. Provide patient education materials and contact information.
• Document new navigation patients on Accession List in SharePoint and in Athena EMR. • Sends “patient cases” in Athena EMR to communicate with providers who have ordered the breast imaging or with breast surgeons as appropriate. • Discharges patients as appropriate. Documents discharges both negative and positive on Accession list and move to discharge list in Sharepoint. • Positive Results - Alert nurse navigator and work with nurse navigator to ensure a smooth transition. Add patient name to positive Accession list. • Book an appointment for the patient If they were biopsied by the radiologist and needs to see a breast surgeon.
• Provide referral to the Breast Nurse Navigators for those patients diagnosed with cancer, or those with complex situations, so that nursing care and referrals for breast cancer patients and family occur taking into consideration physical, psychological, and emotional needs. • Keep tickler file system updated daily, checking Athena several times during the day for new updates. Go through patient files, checking for appointment completion, biopsy completion and pending pathology reports. Move patient files using established ticker system, documenting progress on the file. • Navigate patients from all breast imaging locations, as assigned. • Implement remote navigation workflow as indicated by Supervisor.
For patients with other breast issues in need of consultation: • Meet with patients who have other breast issues such as cysts that need aspiration, dense breast, family history or other concerns. • Offer the patient a surgical consultation appointment. Schedule an appointment for the patient at a convenient time for the patient. • Provide contact information if we can be of any help to patient in future. • Offer information on breast self-awareness and genetic counseling as needed. • Document this encounter in designated log book, unless patient is a BI-RADS 4 or 5 patient who is navigated.
General Job functions: • Answer the Breast Navigation phone line, providing assistance, or refer phone calls as appropriate. Understand the appropriate contacts for referral of the patient, including when to refer to the Breast Center Nurses or Nurse Navigators. Pick up voice messages on the Breast Navigation line. • Understand the navigation workflow and direct the patient to health care services within Summit Health, at outside facilities, and within the community. • Create a file for each new navigation patient according to established system.
• Manage tickler file system on a daily basis, checking for new pathology results, appointments scheduled and chart notes. Update tickler system accordingly.
• Coordinate and manage patients across the continuum of care beginning with the diagnostic mammography/or ultrasound through the diagnosis of cancer, coordinating care with the Breast Care Center and Radiology.
• Monitor usage and prepare new patient binders and folders in advance and take inventory of patient education materials. • Coordinate the availability of binders and patient education materials as well as being familiar with resources available in the community. Order as needed. • Add email addresses to the Accession list once patient is aware of their diagnosis as indicated by the Nurse Navigator. • Ensure that patients are marked appropriately on the accession list after patient is navigated, so that patient can be surveyed by NRC. • On a weekly basis, report number of new imaging navigation patients on designated spreadsheet, both for surgical consult and radiology biopsy referral. • On a monthly basis, provide Navigation Statistics for assigned sites, having the Nurse Navigator review before submitting as appropriate. • Ensure that Accession List is fully populated for all new patients and updat
