Anticipated End Date: 2026-09-04 Position Title: Outreach Care Specialist (US)-7 Job Description: Outreach Care Specialist Location: Tampa or Miami, FL Schedule: Monday-Friday 8:30am-5pm
Hybrid 1 (1 day): T is role requires associates to be in-office 1 days per week, fostering collaboration and connectivity, while providing flexibility to support productivity and work-life balance. This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace.

The Outreach Care Specialist is responsible for ensuring that appropriate member treatment plans are followed on less complex cases and for proactively identifying ways to improve the health of our members and meet quality goals.
How you will make an impact: The person in this role will work directly with members who have housing and employment needs, providing hands-on outreach, coordination, and ongoing support. Day-to-day responsibilities will include: • Conducting proactive outreach to members by phone and through community-based engagement to understand their housing, employment, and other social support needs. • Building relationships and establishing trust with members, including those who may be difficult to reach or experiencing barriers to accessing services. • Assessing individual member needs and identifying barriers related to housing stability, employment, and access to community resources. • Connecting members with appropriate community-based organizations (CBOs), housing resources, employment programs, and social-service agencies. • Coordinating referrals and following up with both members and community partners to help ensure members successfully access the services they need. • Providing ongoing case management support, including tracking member progress, addressing new barriers, and helping members navigate available resources. • Developing and maintaining relationships with community organizations and other partners that provide housing, employment, and supportive services. • Collaborating with internal care management and other cross-functional teams to coordinate support based on the member’s individual needs. • Maintaining timely and accurate documentation of member outreach, referrals, interventions, and outcomes. • Identifying gaps or recurring barriers members experience and sharing opportunities to improve processes, resources, and overall member outcomes.
• Administrative stretch work to assist with tracking member utilization and program outcomes and more.
Minimum Requirements • High school diploma or equivalent, plus at least 1 year of related experience, or an equivalent combination of education and experience.
Preferred Skills, capabilities and experience: • BS/BA degree in a related field preferred. • Bilingual candidates preferred. • For URAC accredited areas, the following professional competencies apply: Associates in this role are expected to have strong oral, written and interpersonal communication skills, problem-solving skills, facilitation skills, and analytical skills. For Carelon - CareBridge business unit, bilingual or multi-language skills may be required. • Case management or care coordination experience, particularly supporting individuals with housing and/or employment needs. • Experience conducting community-based outreach and working directly with members in the community. • Experience partnering with community-based organizations (CBOs) and connecting members to local resources and social services. • Familiarity with housing, employment, workforce development, or social-service programs. • Previous healthcare, care coordination, social services, or member/patient-facing experience. • Strong verbal and written communication skills. • Strong interpersonal skills and demonstrated ability to build rapport and trust with members.
• Strong problem-solving skills, particularly around identifying and addressing barriers to housing, employment, or other services.
• Ability to coordinate multiple member needs, referrals, resources, and follow-ups.
• Comfortable conducting proactive member outreach and engaging individuals who may need additional support navigating services.
• Strong documentation, organizational, and follow-through skills.
