Organization: VHCS Clinical - Pediatrics Job Summary: Provide pediatric occupational therapy services to patients who have impairments, functional limitations, physical/developmental disabilities or changes in physical function and health status resulting from injury, disease or other causes. Actively collaborates with the health care team to meet patient care, outcomes and system improvement objectives. . Vanderbilt Health is seeking a compassionate Occupational Therapist to join our Pediatric Home Health team serving Davidson and Williamson Counties. In this role, you'll provide individualized, evidence-based therapy services to children from birth through age 18 in their homes while enjoying a flexible schedule and exceptional work-life balance.

Working alongside a multidisciplinary team, you'll help children build independence, improve functional skills, and achieve developmental goals in the environment where they live and grow.

Responsibilities

• Provide occupational therapy services to pediatric patients in their homes

• Evaluate developmental, sensory, motor, and functional abilities

• Develop and implement individualized treatment plans

• Educate caregivers on therapeutic techniques and home programs

• Collaborate with interdisciplinary team members to coordinate care

• Complete timely clinical documentation and EMR charting

• Participate in case conferences and care planning meetings

• Support children with developmental, neurological, orthopedic, and medically complex conditions

Qualifications

Required

• Graduate of an accredited Occupational Therapy program

• Tennessee Occupational Therapist license

• CPR certification

• Valid driver's license and reliable transportation

• Strong communication, organization, and documentation skills

Preferred

• Pediatric Home Health experience

• Acute pediatric experience

• Experience working with medically fragile infants and children

• EMR documentation experience

KEY RESPONSIBILITIES

PLANNING AND MANAGING CARE • Formulate functional treatment plan that is goal focused, individualized, safe and age-specific to address deficits and patient/caregiver areas of concern.

• Evaluate patient/caregiver responses and modify plan of care according to VHCS and VUMC standards.

• Administer and score standardized and criterion referenced tests.

• Interpret the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team.

• Evaluate and incorporate new research findings into the assessment and treatment of patients.

• Assist with establishing or modifying existing "Standards of Practice" for specific patient groups.

• Complete documentation according to agency policy.

• Meet productivity standards.

• Demonstrate accountability as an individual and team member in meeting patient/family needs

• Participate in identifying cost effective measures for clinical care.

• Manage clinical assignment in an organized, efficient manner using human and other resources.

• Continuum of Care/Provision of Care

• Provide therapeutic interventions for identified objectives.

• Evaluate patient/caregiver responses and modify plan of care according to VHCS and VUMC standards.

• Assure coordination of care for all patients including multi-disciplinary coordination as a case manager.

• Identify and recommend for referral to other services as indicated.

• Assure patient safety.

• Establish relationship of trust and confidentiality with patient/caregivers.
