Company: Prima Medical Foundation dba MarinHealth Medical Network Work Shift: 8 Hour (days) (United States of America) Scheduled Weekly Hours: 40 Job Description Summary: The Nurse (RN) - Navigator, Cardiology, Structural Heart provides care coordination from referral and test scheduling through treatment options for patients undergoing Transcatheter Aortic Valve Replacement (TAVR), Left Atrial Appendage Closure (LAAC – Watchman and Amulet), Transcatheter Edge-to-Edge Repair (TEER – MitraClip), and Patent Foramen Ovale/Atrial Septal Defect (PFO/ASD) closures.
The Nurse (RN) - Navigator, Cardiology, Structural Heart reports to the Clinical Program Director for Cardiology and is responsible for providing patient education, coordinating care with physicians and surgeons across disciplines, and meeting documentation and compliance requirements. Responsibilities include nursing assessment, problem identification, planning, reporting complex clinical data to the appropriate cardiologist, and implementing the patient’s plan of care as directed by the cardiologist.
In addition, this role manages transitions of care from the acute setting to home, supports Care Team duties as assigned, and maintains patient follow-up in accordance with Centers for Medicare & Medicaid Services (CMS) and National Cardiovascular Data Registry (NCDR) guidelines. Job Requirements, Prerequisites and Essential Functions: Pay Range: $62.78 - $78.47 - $94.16
Job Specifications: Education: • Bachelor's Degree of Science in Nursing from an accredited college/university
Experience • Minimum of two-year cardiovascular clinic experience and/or hospital nursing experience.
• Two years critical care hospital experience preferred.
• Two years case management experience in the hospital or clinic setting preferred.
• Excellent written and oral modes of interpersonal communication.
License and Certifications: • Maintains CPR and ACLS certification.
• Current Registered Nurse California
Duties And Responsibilities:
Essential (Not Modifiable) • Maintains patient referral and database using Microsoft Excel and TEAMS, and maintains compliance documentation for insurance and the NCDR.
• Educates patients about their cardiovascular condition and explains treatment options as directed by the cardiologist.
• Teaches patients what they can do to prevent the worsening of cardiovascular conditions, the importance of self-monitoring and provide appropriate resources.
• Triages patients’ physical symptoms / arrhythmias appropriately and consults with cardiologists per protocol.
• Participates in Transition of Care Management to ensure discharge medications and instructions are understood by patient and/or family members. Updates medication and problem list in the Electronic Health Record and ensures follow-up with cardiologists.
• Reviews physician tasks and clinical worklist(s) as assigned.
• Reviews preoperative lab and imaging results when applicable and ensures appropriate management of results per MD.
• Assist the Clinical Program Director and Structural Heart Team on matters relating to nursing practices and participates in development of policies and protocols for patient care.
• Assists in maintaining safety equipment / emergency cart within the clinic.
• Maintains and helps to ensure that all documentation is in a state of audit-readiness.
• Promotes ongoing communication and collaboration with the clinical manager, physicians, and appropriate staff to ensure patient safety and adherence to clinic protocols.
• Takes initiative to identify and provide solutions to work-related problems, continuously seeking ways to improve services.
• Covers for other staff as deemed necessary both inter and intra departmentally.
• Updates Cardiology Leadership of changes affecting their schedule.
• Other duties as assigned.
Team • Responds to emergencies and supports Team in acute situations.
• Assists in maintaining safety equipment / emergency cart within the clinic.
• Maintains and helps to ensure that all documentation is in a state of audit-readiness.
• Promotes ongoing communication and collaboration with the clinical manager, physicians, and appropriate staff to ensure patient safety and adherence to clinic protocols.
• Takes initiative to identify and provide solutions to work-related problems, continuously seeking ways to improve services.
• Occasionally covers for other staff as deemed necessary both inter and intra-departmentally.
• Updates Clinical Lead of changes affecting their schedule.
Practice • Professional representative within the clinic and the community
• Participates in clinic meetings as required
