About this role

Western Home Communities is seeking an experienced, organized, and compassionate LPN or RN leader to join our team as a Nurse Mentor at our Madrid location.  In this unique role, you'll lead with heart and excellence, empowering staff, supporting residents, and setting the standard for high-quality, person-centered care. What You’ll Do: • Provide direct resident care while mentoring and supporting all levels of care staff

• Identify and manage infection control practices; educate team members on proper precautions

• Oversee and participate in MDS processes — including scheduling and transmitting assessments

• Supervise and guide direct care staff to ensure the highest standards of care are met

• Administer medications, treatments, and physician-ordered care with precision and accountability

• Communicate effectively with physicians, supervisors, and family members regarding resident needs and progress

• Foster a team-oriented, compassionate, and professional environment

What You’ll Bring: • Graduate of a state board-approved LPN (Licensed Practical Nurse) or Registered Nurse (RN) program

• Current Iowa LPN or RN license in good standing

• Strong organizational, communication, and leadership skills

• Preferred: Familiarity with MDS process

• Preferred: Certification in Infection Prevention and Control (or willingness to obtain)

• Completion of the Iowa mandatory supervisory course (or willingness to complete within 6 months)

• Current CPR certification (or willingness to obtain)

• Flexibility to work weekends and holidays as needed
