Position Summary
The Nurse Care Coordinator Lead (RN) is an integral member of Clarity Pediatrics' interdisciplinary care team, collaborating with physicians, advanced practice providers, psychologists, and therapists. Under the direction of clinical leadership, the Nurse Care Coordinator lead ensures seamless care delivery for children with chronic conditions by coordinating clinical appointments, providing patient and family education, managing medication authorizations, and serving as a primary point of contact between families, providers, and pharmacies. This role will also be the supervisor for the team of nurses, medical assistants, and community health workers. Note this is a fully remote position.
Key Responsibilities
LEADERSHIP RESPONSIBILITIES INCLUDE:
STAFF SUPERVISION & DEVELOPMENT
- Recruit, hire, train, and onboard new team members
- Provide direct supervision and performance management for all assigned staff
- Conduct regular 1-on-1 meetings, performance reviews, and coaching
- Identify professional development needs and facilitate training and continuing education
- Address performance issues promptly and constructively; document as appropriate
- Prepare and deliver staff schedules and manage coverage
OPERATIONAL MANAGEMENT
- Maintain compliance with healthcare regulations (HIPAA, state nursing boards, etc.)
- Ensure timely and accurate documentation and record-keeping
- Participate in quality improvement and process optimization initiatives
- Track and report key performance indicators and metrics
- Coordinate with other departments to ensure seamless patient care transitions
- Maintain appropriate staffing ratios and coverage
COMMUNICATION & COLLABORATION
- Communicate effectively with patients, families, and interdisciplinary team members
- Participate in team meetings and clinical huddles
- Serve as liaison between clinical staff and management
- Report to leadership on team performance, concerns, and successes
- Collaborate with community partners and external resources
Also assist with coverage and support of Care Coordination Team duties, including:
Care Coordination & Scheduling
- Coordinate, schedule, and reschedule clinical appointments across the care team
- Manage referrals to specialist and external clinical services
- Proactively follow up with families to reduce gaps in care and no-show rates
Communication
- Provide clinical updates to referring physicians
- Coordinate lab work and documentation requirements for treatment planning with referring providers
- Support the providers for patients requiring escalation of care
- Provide evidence-based education to families regarding chronic condition management
- Explain medication regimens, treatment plans, and care goals in accessible language
- Respond to patient and family inquiries via messaging and phone in a timely manner
Medication Management & Pharmacy Coordination
- Initiate and manage medication prior authorization requests with insurance payers
- Communicate with pharmacies to resolve dispensing issues and coordinate refills
- Track authorization status and escalate delays to the appropriate clinical team member
- Maintain accurate medication records and document all pharmacy communications
Qualifications
Required:
- Licensure: Active California Registered Nurse (RN) license in good standing (Associate's, Bachelor's, or Master's degree in Nursing accepted)
- Leadership: Experience leading a team of clinical staff
- Tech-Savvy: Demonstrated proficiency with multiple technology platforms and electronic health record (EHR) systems
- Communication: Strong organizational, communication, and interpersonal skills
- Virtual Environment: Ability to manage competing priorities and work collaboratively in a remote, virtual environment
- Cultural Responsiveness: Comfort working with diverse, medically and socially complex pediatric populations
- Startup Mindset: Excitement about joining a fast-growing startup and iterating on new care models.
- Internet Access: Reliable internet connection and a dedicated workspace suitable for remote work
- Bilingual: English/Spanish
Preferred:
- Experience in pediatric, chronic care, or care coordination settings
- Familiarity with prior authorization processes and payer requirements
- Experience working in underserved or urban environments
Logistics
- Full-time, contract position (40 hours per week to start)
- Due to our clinic hours, this position will be M - F from 9 AM - 6 PM PST with a 1 hour break daily
- Occasional Saturday coverage (to be decided in advance)
- $90,000 - $110,000/year with medical and ancillary benefits (depending on Geographic Location)
