*** Limited Term Up to 1 Year ***

POSITION SUMMARY: The Field LVN provides skilled nursing care to patients in their homes.  Care is delivered following a plan established by the physician and the Team Leader/Case Manager in conjunction with the patient/family and in compliance with the California Vocational Nurse Act.  The LVN works within the framework of the team.

POSITION QUALIFICATIONS: 

Education/Licensure/ Certification:

• Current California Licensed Vocational Nurse (LVN) license with one year’s experience as a practicing LVN required.  • Current Basic Life Support (BLS) certification required.  • IV certification documentation prior to employment or completion during orientation period is required. • Must meet all DMV requirements listed below and have adequate transportation.

Knowledge/Skills: • Effective communication skills in both written and oral forms in English. • Must have good organizational skills and be self-directed.  • Able to draw blood, provide wound care, and educate patients.
