As a Health Navigator, you'll serve as an advocate, resource coordinator, and trusted partner for individuals navigating complex healthcare systems. You'll help clients access insurance coverage, connect with primary and specialty care providers, obtain medications and transportation resources, and remain engaged in medical and behavioral health treatment.

Key Responsibilities: • Build meaningful relationships with clients and encourage ongoing engagement in healthcare and support services • Assist eligible individuals with enrollment in Medicaid and other insurance programs • Connect clients to primary care physicians, specialty providers, behavioral health services, pharmacies, transportation resources, and other community supports • Coordinate referrals and monitor follow-through with healthcare appointments and treatment recommendations • Provide case management and care coordination services to support individual health and wellness goals • Advocate for clients as they navigate healthcare systems and community resources • Develop and maintain strong partnerships with healthcare providers, insurance representatives, community organizations, and referral sources • Conduct outreach and re-engagement efforts for individuals who have disconnected from medical or behavioral healthcare services • Track client progress, referrals, outcomes, and service utilization while maintaining accurate documentation • Support organizational initiatives designed to improve healthcare access and population health outcomes • Represent the organization professionally while promoting positive community relationships • Perform other duties as assigned to support program success and client care
Education, Experience, and/or Credential Qualifications: • Bachelor's degree in Social Work, Counseling, Psychology, Psychiatric Nursing, Human Services, Healthcare Administration, or a closely related field OR • Minimum of four (4) years of relevant full-time experience in human services, healthcare navigation, case management, care coordination, community outreach, or a related field OR • An equivalent combination of education and qualifying experience, with experience substituting on a year-for-year basis • Demonstrated ability to build positive, productive client relationships and effectively navigate complex service systems • Basic computer proficiency, including familiarity with Microsoft Word, Excel, Outlook, and electronic documentation systems • Experience working with individuals experiencing mental health conditions, substance use disorders, chronic health conditions, or social determinants of health preferred
Additional Qualifications: • Current driver's license, acceptable driving record, and reliable transportation required • Strong communication, relationship-building, and advocacy skills • Ability to work independently while collaborating effectively with multidisciplinary teams and community partners • Knowledge of healthcare systems, insurance programs, community resources, and referral processes preferred • Strong organizational skills with the ability to manage multiple priorities and deadlines • Ability to maintain confidentiality and exercise sound professional judgment • Passion for helping individuals overcome barriers and achieve improved health outcomes • Commitment to person-centered, trauma-informed, and recovery-oriented service delivery • Required certifications, trainings, and professional development activities must be obtained and maintained according to organizational policy
Physical Requirements • Perform sedentary work involving prolonged periods of sitting, computer use, and documentation • Frequently communicate with clients, providers, and community partners in person, by telephone, and through electronic communication • Occasionally exert up to 10 pounds of force to lift, carry, push, pull, or move objects • Travel within the community as needed to support client engagement, care coordination, and partnership development • Move throughout office and community environments as necessary to perform essential job functions
