The University of Vermont Medical Center   is seeking an experienced   Nurse Practitioner   to join our Family Medicine team in   Colchester, Vermont .

At Colchester Family Medicine, relationships matter. Join an established, collaborative team providing comprehensive primary care to patients and families across the lifespan.

Schedule • 0.75 FTE (30 hours/week)  

• 24 hours of direct patient care with dedicated administrative time

• Daytime schedule, Monday through Friday

• Dedicated administrative time, with remote work opportunities available based on department needs

• Quarterly   Saturday   weekend clinic participation following orientation

What You'll Do • Provide comprehensive outpatient primary care for patients across the lifespan

• Manage and grow your own patient panel within an established Family Medicine practice   with strong physician and APP collaboration

• Diagnose and treat acute and chronic medical conditions

• Deliver preventive care and wellness services

• Conduct in-person and telemedicine visits

• Collaborate with an interdisciplinary care team to support exceptional patient outcomes

Experience • Minimum of two years of Nurse Practitioner experience in Primary Care, Family Medicine, or a related outpatient setting required

• Experience providing care across the lifespan   required

• Commitment to patient-centered, team-based care   in an academic medical center 

Education & Licensure Requirements • Completion of an accredited Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) program

• Current National Board Certification as a Nurse Practitioner

• Active Vermont APRN license or eligibility for Vermont licensure

• Eligibility for NY licensure 

• Eligibility for prescriptive authority

• Eligibility for DEA licensure 

Telemedicine opportunities are available. Providers must be eligible to obtain both Vermont and New York licensure to support virtual care for patients located in New York.
