EPC Psych Case Manager

About this role

Job Description: The EPC Psych Case Manager position will receive referrals from both inpatient and outpatient services. Clients can be either EPC or voluntary referrals. This person will be responsible to p erform an Intake Assessment and Individualized Program Plan for each client. Provide on-going support, advocacy, and crisis management follow-up services for referred patients. Length of service may be up to six months in duration.

EDUCATION : Bachelors Degree in a Mental Health related field (Social Work, Psychology, etc.) required. EXPERIENCE : Preferred 2 years of clinical experience in case management related activities for persons with severe and persistent mental illness. Preferred clinical experience with substance abuse treatment issues. CERTIFICATIONS/LICENSES : Valid NE Drivers License required. Basic Life Support (BLS) through American Heart Association (AHA) required within 3 months of hire. CPI required within 3 months of hire.
