Organization: MCJCHV Nutrition Support Srvcs Job Summary: JOB SUMMARY Assesses nutrition status of patients, including performance of a nutrition-focused physical exam, and identifies nutrition diagnoses. Develops, implements and evaluates outcomes of nutrition care plans for patients at nutrition risk. Initiates changes in care plans as needed. Provides nutrition education and counseling to patients, families, and the community. Trains dietetic interns and other health care students. .
KEY RESPONSIBILITIES • Assesses nutrition status, including performance of a nutrition-focused physical exam, and implements nutrition care plans to address identified nutrition diagnoses for patients with common medical conditions, using the Nutrition Care Process and standardized nutrition terminology in documentation. • Identifies patients who meet the criteria for a medical diagnosis of malnutrition and documents criteria in the medical record. • Writes, accepts, and implements medical nutrition therapy and nutrition orders in eSTAR, including diet and infant feeding orders and enteral nutrition orders, according to the VUMC Standard Operating Procedure and RD/LDN Scope of Practice. • Monitors nutrition progress and reassesses nutrition status of patients/clients with common medical conditions, adjusting the nutrition care plan when needed. • Provides nutrition counseling and education to patients, families and caregivers, and the community, using evidence-based nutrition practice guidelines and appropriate instructional methods and tools. • Conducts nutrition telehealth, real-time HIPAA-compliant interactive audio and video telecommunications with patients, and monitors telehealth technologies for HIPAA compliance. • Trains dietetic interns and other students by planning, implementing, supervising and evaluating clinical experiences and/or teaching classes.
TECHNICAL CAPABILITIES • Patient Counseling (Intermediate): Demonstrates mastery of patient counseling in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering counseling services without requiring support and instruction from others. Able to train and educate by setting the example, giving technical instruction, providing leadership, and generally raising the level of performance of others while on the job. • Nutrition Therapy (Intermediate): Demonstrates mastery of nutrition therapy in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering results without requiring support and instruction from others. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job. • Dietetic Patient Assessment (Intermediate): Demonstrates mastery of patient assessments in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering results without requiring support and instruction from others. Conducts patient interviews, physical examinations, and data collection in order to accurately identify nutrition problems and to contact the patient's care provider related to nutrition interventions. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job. • Evidence-Based Nutrition Practice (Intermediate): Demonstrates mastery of integrating nutrition evidence from multiple sources to determine if a practice change should occur. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering evidence-based nutrition practice without requiring support and instruction from others.
