Job Details
The CareKinesis Clinical Initiatives Pharmacist utilizes clinical knowledge and skills coupled with innovative technology to optimize medication regimens and prevent adverse events for participants by identifying and rectifying medication-related problems, empowering participants to actively participate in their own clinical decision-making, and collaborating with providers, pharmacists, and other healthcare providers to improve outcomes.
Responsibilities • Conduct comprehensive and targeted Medication Safety Reviews (MSRs) for PACE participants using proprietary clinical platforms, developing individualized, measurable, and goal-oriented therapeutic care plans.
• Complete medication reviews and reconciliation activities for new enrollments, annual and significant change assessments, care transitions, and other high-risk clinical events to support safe and effective medication use.
• Identify, evaluate, document, and facilitate the resolution of medication-related problems, including polypharmacy, adverse drug events, potentially inappropriate medications, therapeutic gaps, adherence challenges, and medication access barriers.
• Collaborate with providers, interdisciplinary team (IDT) members, and other healthcare professionals to communicate clinical recommendations, including medication optimization, deprescribing opportunities, and goals-of-care aligned interventions when appropriate.
• Engage with providers through written medication safety reviews, clinical consultations, polypharmacy assessments, fall risk evaluations, and care transition reviews, while monitoring and documenting outcomes and progress toward established therapeutic goals.
• Monitor participants receiving high-risk medications or managing complex chronic disease regimens, ensuring timely follow-up, intervention, and documentation of clinical findings and recommendations.
• Participate in client meetings, interdisciplinary discussions, Pharmacy and Therapeutics (P&T) Committee meetings, drug utilization review activities, quality improvement initiatives, and educational programs as assigned.
• Maintain compliance with all applicable federal and state regulations, accreditation standards, and organizational policies governing pharmacy practice, including requirements related to quality assurance, HIPAA, Fraud, Waste and Abuse (FWA), and patient safety reporting.

Education • Graduate of a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction.
• A current, active license/registration in good standing with the state board of pharmacy where practice will occur.
• Active Board of Pharmacy Specialties (BPS) certification in geriatrics (BCGP) preferred.
Experience • Experience in direct contact with prescribers and patients is preferred.
• Experience or strong interest in clinical pharmacogenomics is preferred.
• Above-average knowledge of geriatric pharmacotherapy and ability to apply knowledge to clinical practice including (1) focusing on the special needs of older patients who may have concurrent illnesses taking multiple medications and (2) resolve complex medication-related issues that arise in the geriatric patient population.
• Strong analytical skills with good clinical judgement and decision making including the ability to effectively and efficiently identify and rectify medication-related problems.
• Self-motivated and eager to develop clinically and professionally with a high degree of ownership for tasks and self-accountability and strong follow-through.
Skills & Abilities • Ability to communicate effectively and professionally through written, verbal (e.g., face-to-face, telephonically and/or virtual communication platforms), and interpersonal skills as applied when interacting with employees, patients, healthcare professionals, clients, or agency representatives; successfully conveying and exchanging information at an appropriate level and in a positive and timely manner.
• Strong technical and computer skills to navigate proprietary and other pharmacy software and the initiative to propose suggestions to improve operations and proprietary system development and enhancements.
• Demonstrated experience with problem-solving, conflict resolution, and project management skills.
• Detail oriented and organized with the ability to work with a sense of urgency, work under pressure, multi-task, and achieve deadlines without sacrificing quality.
