Role Scope : The PACE Center Manager oversees the daily operations of the Habitat Health PACE Center to ensure safe, efficient, and participant ‑ centered delivery of services. Reporting to the Center Executive Director, the Center Manager leads and supports clinical, social, and operational staff, ensuring smooth coordination across all center ‑ based workflows. This role is responsible for maintaining regulatory compliance, supporting interdisciplinary team (IDT) processes, and ensuring participants experience high ‑ quality care while at the Center. The Center Manager partners closely with the Medical Director, Clinical Services Director, and Practice Operations leadership to meet performance goals, uphold quality standards, and foster a positive, collaborative environment for participants and staff.
Core Responsibilities & Expectations for the Role
• Oversee daily PACE Center operations, including participant flow, staffing, scheduling, and coordination of services to ensure safe and efficient care delivery.
• Participate as an active member of the IDT and support other members by ensuring timely assessments, care planning activities, documentation, and collaboration across clinical and social care disciplines.
• Maintain compliance with PACE, CMS, DHCS, ADHC, and state regulations, including monitoring documentation accuracy, regulatory timelines, and audit readiness.
• Lead and support center staff through coaching, performance management, workflow guidance, and fostering a culture of accountability and teamwork.
• Monitor operational and quality performance metrics, identify gaps, and implement improvements to meet Center level goals and service level agreements (SLAs).
• Address operational barriers, participant concerns, and workflow issues in real time, escalating complex challenges when appropriate.
• Maintain a safe, organized, and welcoming Center environment, ensuring adherence to safety protocols, infection control standards, and emergency preparedness requirements.
• Partner with the Medical Director, Clinical Services Manager/Director, and Practice Operations leadership to align daily operations with organizational priorities and participant needs.
• Perform related duties as assigned to support smooth Center operations and high ‑ quality participant experience.
Required Qualifications:
• Bachelor’s Degree Health Care Administration, Business Administration, or Public Health or equivalent work experience.
• Three (3) to five (5) years of experience leading teams in a senior care services area working with elderly population.
• Ability to work cross-functionally with clinicians, leaders, stakeholders and communicate effectively at all levels.
• Strong decision-making skills with ability to prioritize needs across the Center, plan and execute scalable processes.
• Proven ability to lead strong teams, manage and drive performance, and deliver on organizational and team goals effectively
