The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).
Pay Range: $41.60 - $54.63 Scheduled Weekly Hours: 36 Position Overview SHIFT: 6:00pm-6:30am (rotating weekends and holidays)
ESSENTIAL FUNCTIONS • Coordinates the overall functions of the Patient Logistics department and Transfer Center in conjunction with the Manager. Coordinates bed flow and transfer center functions through the management of the capacity management board as well as the CAD system. • Provides daily operational oversight and decision making of patient flow activities throughout Capital Health by collaborating with Patient Access, Emergency and Surgical Services departments, as well as physician offices, in assessing, planning and implementing patient flow activities to promote patient and customer satisfaction. • Communicates with clinical unit staff regarding transfers and admissions and facilitates timely movement of the patient. • Collaborates with the physician, care manager, and floor staff to ensure timely coordination of discharge and transfer. Communicates and collaborates with physicians regarding transfer orders as needed. • Assesses patient status/acuity to facilitate the efficient movement of patients through the hospital system by prioritizing inpatient admissions, critical care admissions, and internal/external transfers. • Serves as an expert clinical resource for patient placement by ensuring smooth functioning of the Logistics Center process to reduce wait times and avoid unnecessary movements of patients. Serves as contact person for capacity management as it relates to admissions and transfers. • Ensures throughput on units and compliance with transfer process through management of people and process. Delegates tasks appropriately and employs resources as needed to ensure high quality and efficient care related to patient flow. • Identifies and anticipates barriers to discharge/patient flow activities and works to ensure throughput in ED, inpatient units and compliance with transfer process through management of people, metrics and process improvement. • Activates the Hospital Surge/Capacity plan based on the guidelines established and works to initiate/activate/manage/close identified overflow patient care areas. Reviews blocked and closed beds daily to complete daily closed bed report with bed numbers and reasons for closure. • Monitors inpatient bed status and upgrades bed cleaning status via bed tracking system to expedite patient flow to a clean bed. Coordinates with Transport and Environmental Services supervisors to expedite patient flow. • Monitors patient movement within and outside CH to ensure timely and efficient transfers of patients to avoid additional unnecessary length of stay. • Coordinates the transport of patients to and from Capital Health facilities. Coordinates the transfer of patients that need emergent or non-emergent Neurological, Trauma, Cardiac or other specialized medical services at Capital Health. • Facilitates physician to physician communications for admission to a Capital Health facility or out of Capital Health for transfer. • Dispatches and tracks Wheelchair, BLS, and SCT services using computer aided dispatch (CAD) or outsource requests to contracted agencies. Requests Uber transportation for discharged patients. • Coordinates with the County 9-1-1 Communications Center and Capital Health EMS units or supervisors. • Receives updates from Capital Health facilities reporting diversion status and reports the status to various County and State agencies. • Communicates openly and effectively by providing ongoing education to the health care team on the overall patient flow strategy on rationale for patient placement. • Uses effective and appropriate communication styles with colleagues. Promotes team momentum, enthusiasm and pride. • Functions as a department team member to support the unit and Capital Health’s goals, objectives and established policies. • Performs other duties as assigned.
MINIMUM REQUIREMENTS • Education: Associate's degree and graduate from accredited school of nursing. • Experience: Four years experience in an acute care hospital setting to include minimum Telemetry experience. Critical Care experience preferred. • Other Credentials : AHA BLS - Healthcare Provider,Registered Nurse - NJ • Knowledge and Skills: Strong verbal and written communication skills. Excellent customer service orientation skills necessary to communicate effectively and utilize effective conflict resolution and negotiation skills to support a cohesive multidisciplinary team and ensure optimal patient outcomes. Possesses ability to use critical thinking, logic and reasoning to address complex situations. Demonstrates responsiveness to needs of patients, other care team members, and self. Ability to organize and prioritize, in a fast-paced environment.
