- Fast Paced- Startups often must move faster than traditional large companies. This means you may be asked to cope with or advise on quick changes regarding systems or processes
- Autonomy- You will need to be highly autonomous, as there is less structure and learning and development than at large orgs. This requires you to be able to learn asynchronously, as well as raise your hand when help is needed, or gaps are identified
- Collaboration- With so many opportunities to build processes and improve how we deliver care to our patients, we rely heavily on one-another to communicate changes effectively to ensure seamless support across functions. This often requires this individual to give and receive feedback frequently and communicate challenges or opportunities effectively.
WHAT WILL YOU DO?
· Manage a panel of families as their primary care provider, delivering comprehensive longitudinal primary and preventive care, care coordination, and patient education on conditions and medications
· Conduct adult and pediatric wellness visits and acute and chronic care requiring physical examination and diagnostic testing in a community facility setting
· Partner with an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists and medical assistants who support social services navigation
· Attend daily team huddles and weekly case conferences to advise on physical health needs and establish health goals
· Reconcile and prescribe medications, order and administer labs and diagnostics, and refer to specialist care, as needed
· Document care and maintain clear, comprehensive care plans addressing patient and family needs.
· Help families identify and access health and social care resources, including food, housing, transportation, and financial assistance
· Participate in community outreach efforts, as needed
WHAT DO YOU BRING TO THE NEST?
· Graduate of an accredited Nurse Practitioner or Physician Assistant program with an active, unrestricted Arizona license
· At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions (preferred)
· Proven skills, knowledge base, and judgment for independent clinical decision-making and problem-solving
· Hands-on clinical skills, including venipuncture, patient assessment, and chronic care support, or willingness to learn
· Ability to work full-time, including rotating virtual on-call coverage on occasional evenings and weekends
· Current BLS/CPR certification
· Passion for serving underserved and vulnerable populations with empathy and respect
· Leadership experience in an interdisciplinary care team (preferred)

WHAT IS REQUIRED (TRAVEL & PHYSICAL REQUIREMENTS)?
This is a hybrid position, providing telehealth care and in-person care at community facilities or in patient homes.
· Travel between work locations throughout Show Low and/or Flagstaff, including current and future service areas, may be required in emergent situations based on business needs
· Must have a driver’s license, a reliable car (mileage is reimbursed), auto insurance, and be willing to follow the Nest’s Motor Vehicle Safety Policy.
· Prolonged periods sitting at a desk and working on a computer.
· Must be able to lift to 25 pounds at times (healthcare supplies).
